Preparing For Your Discharge

Could we have improved your stay? Or what did we do best?
Please fill out the patient questionnaire when it comes to your home.
Your attending physician is the only person authorized to give you permission to leave the hospital. When you leave, we want to make sure you feel prepared. Our staff will begin talking with you about discharge early in your stay.  Before you leave the hospital, you will be given oral and written instructions about treatment, medication and/or convalescent care. Please be sure to keep your copy. If you have questions or concerns, please ask a member of your healthcare team.

If you need special equipment when you return home (such as crutches, walkers, commode chairs etc.) please speak to your physiotherapist, occupational therapist or case manager.

You will also be given information during your stay about arranging an appointment with our onsite advocate to discuss any remaining co-pay or deductible balances.

   
Post-Hospital Care
   
 

Your doctor may determine you no longer need hospital care but still need some help.  He or she may discharge you to a skilled nursing facility or order home care services. A hospital social worker will assist you in making arrangements. Medicare, Medicaid and insurance policies have coverage limitations for skilled nursing facility care, home care and other services and equipment. Don’t hesitate to ask questions.
