Valley Hospital and Medical Center and Deaconess Medical Center have joined forces with the Rockwood Clinic to improve the quality and delivery of healthcare throughout the region.

Together, we’re creating an integrated healthcare delivery system. Though that may sound complicated, the benefit to patients is simple.

Integrating your care

Integrated delivery systems allow for a higher level of individualized care: from an office visit with a family practitioner and a consultation with a specialist to hospitalization for surgery and recovery with physical therapy. When care is integrated across that course of treatment, caregivers are better able to coordinate. The patient is treated as a whole person—not just as a diagnosis or case file.

This kind of care is particularly beneficial for patients with complex or chronic conditions, as it facilitates better cross-disciplinary information sharing. A stroke patient’s family practitioner can more easily consult with the neurologist and pulmonologist who treat the patient while he or she is in the hospital, as well as the physical therapist or home health aide who cares for the patient after discharge.

Additionally, because a shared medical record extends across the system, the patient’s medical history is both comprehensive and accessible. Patients often experience a higher level of convenience and, consequently, patient satisfaction, because providers are linked and better able to guide patients through the healthcare system.
